*** Important information - November 2017 ***

There is currently a national shortage of certain vaccines - we currently have no access to hep A, hep B or adult pneumococcal vaccines and there is also a shortage of tetanus / diptheria and polio vaccines. If you are aware that you will need any of these vaccines then you may wish to try to access them privately through a Travel Clinic to avoid any delay caused by submitting a travel form to us and subsequently being told that we are unable to assist at the moment

Advice or vaccinations relating to foreign travel are a non-nhs service.

Our Practice Nurse team are able to assess your requirements and provide advice to you . This is subject to following the process and timescales below and subject to available appointment time.

A completed travel questionnaire is required at least 6 weeks before you travel to allow sufficient time for the initial assessment and any subsequent appointment to actually receive a vaccination. Travel vaccines are ordered by need and are not stock items - you will be required to pay at the time that your receive the first course of any vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.

Travels vaccinations that are available on the NHS are provided without charge.

To obtain advice or a travel vaccination :

Step 1 Print and complete the travel questionnaire available at the foot of this page or through our reception team. If you have not allowed sufficient time we may not be able to assess your needs and you will need to contact our reception team by telephone for advice.

Step 2 : Having assessed your requirements you will be invited to attend an appointment to receive advice and vaccination if appropriate.

Step 3 : If you are recommended to have a vaccine and agree to that you will be required to pay when you receive the first of any course.


Travel Health Questionnaire - please check before submitting your travel questionnaire that there is at least 6 weeks before you are due to travel. If you have not allowed sufficient time please contact reception to enquire whether we are able to help.

If you have not heard from us one week after submitting your form you can contact us to arrange your first appointment with the practice nurse.
